Sports Physicals for Texas Locations
Next Level Urgent Care offers $40 sports physicals for school aged children in the Houston, Austin, Beaumont, and San Antonio areas! If you don’t have your school’s physical form, you can download from our list below. Remember to bring this form in to your physical appointment. All Next Level locations are open from 9am-9pm, 7 days a week and no appointment is needed to see our providers. Below lists the package options available for sports physicals:
- Level 1 is a basic UIL physical - $40
- Level 2 is a basic UIL physical + extra paperwork or testing - $65
- Level 3 is a basic physical with an EKG - $100
